Question : I have a red lesion or sore on the tip of my penis about the size of a large vitamin pill, and two red lesions on the side of my penis-opposite sides of each other, towards the head. The red lesion has been there for over 12 days, and occured after I had used a condom. I thought it was just a rub mark from being aggressive, but now I am not sure and I have no known allergeis to latex. I have not been tested for any STD's lately and very nervous, but intended to get one in the next day or so as soon as I can get an appointment. Based upon my limited description, could this be herpes? My last earlier encounter was 5 weeks ago, and I didn't use a condom and know better. Thoughts?

Brief Answer:
?? Looks to me more like balanitis sir; doubt this is due to herpes

Detailed Answer:
Hello sir and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. Based on the images attached the presentation looks to me more like balanitis. So I would like to ask the following:

+Did you wash well after using the condom?
+When did the rash appear? Was it after the first time (when you did not use a condom)? Or was it after the second time (when you used a condom)?
+Depending on the second answer, do tell me if you washed your penis after the sexual contact? If yes, then how long after sexual contact?
+Do you have any itching in the region?
+When you did not use the condom, was your partner a known person or a sexual worker? If you do not mind me asking you sir

With the current information at hand, I doubt this could be due to herpes (but you could get tested for herpes for your satisfaction sir), and think this is most likely due to thrush (fungal infection).

I look forward to hearing from you in order to help you further sir. Please do not hesitate to write to me with any further clarifications, I am always here to help you.

Best wishes.

Thank you for your response and my apology in delay to responding to your questions. Please see my responses to your questions:

1) +Did you wash well after using the condom?
--No I did not. I have not used condoms much, but don't subject to risky behavior...mostly oral. I did not receive oral in this encounter.

2) +When did the rash appear? --The next day after the encounter, about 4 hours later. It was very late at night. Was it after the first time (when you did not use a condom)? --No, only after I had used the condom. Or was it after the second time (when you used a condom)? --Yes

3) +Depending on the second answer, do tell me if you washed your penis after the sexual contact? --Not immediately If yes, then how long after sexual contact? -- It was a couple hours

4) +Do you have any itching in the region? None...no itching at all
+When you did not use the condom, was your partner a known person or a sexual worker? --No, not a sex worker. But I have had a sexual worker in the past, approximately 7 weeks back, and I did not use a condom (I realize that is pure stupidity....). If you do not mind me asking you sir

Thanks again, I was tested last Fall - full panel, but not since. I just thought this rash was some tearing due to being rough, but find it odd that it appeared so quickly. I am going to make an appointment tomorrow to see my doctor, any suggestions on what to get tested for?

Brief Answer:
?? Strongly suspecting a penile yeast infection sir; swab can confirm it

Detailed Answer:
Hello once again sir.

Thank you so much for taking time out to respond to each of my queries, I really appreciate your co-operation. Based on the information at hand sir, my current suspected diagnosis is a penile yeast infection which has been brought about due to the damp and moist environment created around your genitalia when you did not wash the region immediately using a condom.

The diagnosis is quite simple, especially with a close up physical examination that can be conducted by your doctor (hopefully tomorrow), but just in case he/she can go about taking a scraping from the penile head (using a swab) and send it to the laboratory for a confirmation, as yeast is very easily identified under a microscope.

Even before the test results return, your doctor can initiate therapy with anti-fungal medication and I am sure you will experience a response, and if at all you do then the diagnosis will be confirmed there and then. I wish you lots of luck and urge you to always practice safe sexual contact sir. I also want you to know that I am always here to help you, no matter what so do not ever hesitate to write to me for any health-related queries, now and in the future.

Best wishes. :)

I appreciate deeply your responses and proposal to my condition. I know the area is not continually "wet" or damp, or does it itch as I indicated, should it be wet? I do have the two blotches on the sides, right below the head...I assume that is the same Should I have the doctor check for a potential STD's? I believe I will... Thanks again!

Or could it be Syphillis?

Brief Answer:
Keep the region dry always, and yes pls request for an STD-panel

Detailed Answer:
Hi sir.

No the area should not be wet, as fungus/yeast thrive in damp/moist conditions, so please do try to keep the region clean and dry.

Yes sir, along with the penile swab scraping you can request for an 8 STD-panel as it will help put your mind and worries to rest sir. :)

Best wishes.
