Brief Answer:
possible allergic issue

Detailed Answer:
Hi and welcome.

In most cases this can be caused by irritation or perianal and gluteal area due to diarrheas, enterocolitis, parasitic diseases or allergy reactions to clothes and creams. Try to avoid sintetic clothes, suspicious creams. Another cause is dermatitis or fungal infection. It may be atopic dermatitis, beginning of psoriasis or similar dermatoses.

If you can upload a photo of the lesions I will be better able to help. You should see dermatologist for further evaluation.

Wish you good health. Regards
