Suggest Treatment For Red Spot And Swelling Behind Thigh

Question : My son had this issue on 26-Jun-2015 and again having red XXXXXXX on one his thigh back side, we applied T-Bact on it but there is no major relief as its having more redness around it with swelling and increased in size.

Could you please let me know the following:

1)What is the reason for having it again on that side ?

2)Shall i continue with the T-Bact cream?

3)What other medicines does he need to get relief?

4)Generally how long does it take to recover and what all blood test needs to be done?

Welcome to the HCM

I completely understand your concerns. Recurrent furuncle ( XXXXXXX as you mentioned) can occur due to infection of hair follicles with Staphylococcal bacteria. The same site as seen in your child may be affected in some cases due to dormant infection or poor local hygiene. Don't worry as it can be easily treated. The duration of recovery depends upon the severity.

You may take the following measures for treatment:
1. Continue T bact as earlier but clean the site with savlon or dettol twice daily before application of T- bact.
2. Give him an antibiotic such as coamoxiclav or Augmentin for 5 days to completely treat staphylococcal infection.
3. For pain and fever if present, you may give him ibuprofen and acetaminophen.

In case, it's associated with pus formation and fluctuant spot, then contact a surgeon as it may require drainage of pus to be treated completely.

I feel giving frequent antibiotics for the treatment is not always advisable as there are side effects too, since he had completed the 5 days course earlier and having the occurrence of this issue is not clear to me.

I think there must be blood tests needs to be carried out to find out the root cause.

Also correct me if its due to humidity & sweating.

What all food/fruits that can be avoided and precautions needs to be taken?

Please provide more details with point wise answers for the issue.

I appreciate your concerns for the repeated use of antibiotics.

1. Since you had mentioned that this time the furuncle has more redness and swelling associated, this suggests that the infection has spread deeper down into the subcutaneous tissue. As thighs have good amount of loose subcutaneous tissue, the infection may progress fast and needs oral or injectable antibiotics along with topical application to achieve a proper control and proper treatment. Restriction of antibiotics is not advisable in such cases. Since your child had previous infection almost 2 months back, so it won't affect him in adversely.
We see such cases frequently in our clinical practice and treatment I am suggesting is the standard way of treating it.

2. You may go for local swab culture and sensitivity test to check for the organism responsible and it's antibiotic sensitivity. Usually it's staphylococcal and sometimes streptococcal infection. This does not have much clinical use unless and until the infection is resistant to the treatment and child is getting multiple boils, abscesses and furuncles or carbuncles.


There is no use of any blood investigation at this point. It won't help in any ways. Blood investigation will be useful if child shows any signs of septicaemia such as high grade fever, lethargy, weakness , anorexia etc. (We advice blood sugar and immunity work up in kids suffering from recurrent severe and resistant skin infections)

3. You may give him Vitamin C rich fruits such as Amla, lemonade, orange etc. They help in immune building. Rest he may take all seasonal fruits and vegetables as a part of healthy balanced diet with plenty of fluids. Avoid excess oil and spices.

4. Obesity, humidity, sweating and poor local hygiene contribute to such problems. So a good bath with antiseptic soap is always good preventive measure in future.
