Question : over the last few days I noticed red spots on my penis head scrotum and thigh area there painless with a slight itch every so often. could it be an STI? or will it clear up over time?

I have reviewed the attached photograph and it looks like candidal balanitis which is a yeast infection.
It can be transmitted sexually through an infected partner.
If I was your treating doctor I would recommend you to apply antifungal cream like clotrimazole cream on the affected areas twice daily for 2 weeks.
Besides this avoid unprotected sex till the time the infection subsides.
