I am so sorry to hear about what happened to you wife. Can you see this tampon? If yes, I will suggest you pull on it gently while trying to push the walls of the vaginal to the sides. If you cannot see it, then i will suggest that you get her to the nearest ER or urgent care for help. This would ensure that you do not cause any damage to her vagina.
