Suggest Treatment For Repeated Vaginal Yeast Infection

Question: I have vulvodynia and am post menopausal. I have been using estrogen suppositories and progesterone pills for the last 8 weeks. Now I have a yeast infection. I need the hormones to combat the vulvodynia, but I get a yeast infection everytime I use hormones for any length of time. This has happened 4-5 times. What can I do? I have had constant unrelenting pain for 2 years.

Brief Answer: DETAILS ARE GIVEN BELOW. Detailed Answer: Hello, Thanks for writing to us. VULVODYNIA is a chronic pain syndrome that affects the vulva area and often occurs without an identifiable cause or visible pathology. Hormonal treatment is best modality to combat this condition. However, hormonal pill is the main aggravating factor of repeated vaginal yeast infection. Therefore, I would suggest you to undergo one CULTURE and DRUG SENSITIVITY TEST of vaginal discharge along with Blood sugar (PPBS) and differential count of WBC to determine better drug treatment. Next thing, you have to ELIMINATE or CORRECT some additional predisposing factors like mental stress, lack of sleep, poor eating habits, using more steroid, weak immune system, Cervicitis, poor genital hygiene etc. To summarize, you have to modify all these things to get effective result. You can consult with your GYNECOLOGIST for proper guidance. TREATMENT: 1) Vaginal suppository with mixed Clindamycin and Clotrimazole can be used for 5-7 days. Supportive oral antibiotics can be given according to your culture test report. 2) Use vaginal moisturizer cream like RepHresh pH gel or VAGISIL cream to avoid repeated infection and maintain vaginal pH. Maintain genital hygiene properly. 3) Wear only cotton clothes. Don't wear any tight fitting cloths or pants to avoid repeated friction/ irritation. Take sound sleep adequately. 4) Take healthy diet as you have mentioned with anti oxidant (like Vitamin -E or C, Selenium, Zinc, Lycopene etc) supplements. Drink plenty of water or juice for better systemic circulation. You must avoid salty, sugary or fatty foods. 5) Avoid chemical cosmetics or soaps/ perfume or scented bath products around genital area. Keep the area mostly dry. Hope, it helps for your information. I will ask for the tests. Is the drug sensitivity test a blood test or vaginal swab test? I have already had a round of Clindamycin. I also do all the other things you mentioned. Should I continue with the vaginal estrogen and progesterone pills? I know that is what is causing the yeast? What about oral HRT? Would that deliver less hormones so I would get yeast infections?

Why do you recommend oral hrt? And do you mean a hrt patch? There is no such thing as a vaginal patch.
