It sounds like you have many conditions that may contribute to your weakness. I will discuss a few and make some suggestions. Please feel free to send me back other questions if you like.
While it is great that your sugars are under control, your very low A1c makes me concerned that you may be having some sugars that are too low. If that is the case then it could contribute to an overall weakness.
Having a pacemaker makes it impossible for you to increase your heart rate with exercise so that can also contribute to fatigue when you try to exercise.
Some of the medications that you are on can also contribute to fatigue.
Is your blood pressure too low, this can also contribute.
Has your thyroid been checked and is is normal?
Have you tried any physical therapy to help you build up strength in your muscles and overcome any deconditioning that you may have.
Are you taking a multivitamin daily, if not that would be a good idea to take a multivitain and also a B50 which is a B complex vitamin with multiple B vitamins in it.
