I have read through your query in detail.
It looks like you have got persistent ring worm infection in the groin.
Since you have not provided detailed information on the cream used, I suppose you have tried the right ones. However, you say that you have not used it the way you should have. The creams containing both steroid and antifungal like clotrimazole or miconazole work best. This may be applied of the skin twice a day. It will start healing in two weeks time and you can apply for about a month.
Apart from this you also must trim your pubic hairs, wear cotton inner clothes and wash your genitals with warm water twice a day and wipe it dry. Please avoid wearing wet clothes.
The ring worm is caused by a fungus. They thrive on moist surfaces, since the skin in the pubic region might be moist due to sweat formation, they infect that area and the infection spreads if not treated. Wearing wet clothes and not wiping your pubic region completely dry creates a favorable condition for the fungus to multiply.
If the infection is not controlled by application of cream for a month then oral antifungal might need to be taken.

I only use it maybe once a week or only when it starts to itch
Please apply cream twice every day

It is important to apply the ointment twice a day after wiping the area dry. This has to be done for about a month. Applying once a week will not treat the infection.
