about 12 days ago I had an intimate encounter with a sex worker she wore only her underwear and was grinding my crotch while I was wearing boxers. this went on for about 5 minute till i ejaculated. 2 days later I noticed a small rough patch on my foreskin that has been causing me discomfort. Could this be a genital herpes outbreak or is it just friction burn. I have heard you can only contract his disease through skin on skin contact but am still a bit worried. thanks

Its a rough patch on the shaft of the penis. as I recall there was no skin on skin contact. Should I get an STD test?

It is likely to be due to friction with given description. Please clean the area with warm water twice daily and apply topical antibiotic like mupirocin.
