Suggest Treatment For Sarciodosis

Question : Hi,

I have been diagnosed with Sarciodosis. Went to different doctors, all said, that I am not even on stage 1 of the disease, so need no medication. I have undergone various X-ray, CBC, ESR. My ESR is at a scale of 14. All my eye test, stomach test, PFT are normal. Not problem have been found in it.

Recently, I have started feeling bodyaches, just for fraction of seconds. Personally, I think too much for every problem, so I feel fatigued, headache, sometimes. Is this related.

I have following questions:
1. Shall I go under medication? The only thing which stops me is I dont want to take steroids and get diabetes as a result of that.
2. Can this disease go by its own.
3. How will I come to know that the disease is progressing.

I am always very tensed because of this. Please help.

Since all your reports are normal, no need to worry much for sarcoidosis.

Answer to your first question.

No need to start medicine if you are asymptomatic and not having even stage 1 sarcoidosis. So relax and avoid stress.

Answer to your 2nd question.

It is difficult to predict behaviour of sarcoidosis at this moment. It is actually too early to say anything about its progress. You need regular follow up for atleast 1 year to know the activity of the disease, whether regressing or static or progressing.

Answer to your 3rd question.

Following are the evidences to support progression of the disease.
1. Worsening symptoms like cough, breathlessness, low grade fever, weight loss, skin lesions etc.
2. Worsening chest x ray and CT thorax.
3. Deteriorating PFT (pulmonary function test).
4. Need for steroid to control the symptoms.
So, if these are seen in sarcoidosis, means your disease is progressive. This is the reason for regular follow up with pulmonologist. So consult pulmonologist every 2 monthly for clinical examination and PFT. CT thorax is required every 6monthly.
