Suggest Treatment For Scabies, Ear Infection And Sore Throat In Children

Question: Hi my question is in regrads to my two boys in XXXXXXX i was told they had scabies and i did treatment and washed everything including carpets but the rash never went away few weeks ago i brought them back to dr office and they were given antibotics for sore throat and ear infections also was given predosione to see if that would help rash, i had already did treatment twice before and they had wanted me to do it another two times which i didnt at first i gave steriod first to see now it was given once a day for 3 days on the secnod day i noticed red bumps going away slowly but after treatment on the 3 day bumps came back and got a little more, i am at a lost as to what this could be no one has in family has gotten this either if u could help me in possibly what this might be would be very helpful to me thanks

I understand your concern.

Patients with scabies may need to be reexamined at 2 weeks and again at 1 month after treatment. If a patient has persistent lesions at the 1-month check-up, reinfection or persistent infection should be suspected.

In this case, treatment should be reinitiated. The patient’s family or any close contacts should also be examined to check for a source of reinfection. Patients with crusted scabies, especially, should be followed after treatment and may require repeated courses of treatment.

All household members and close personal contacts older than 2 months and not pregnant should be treated for scabies, even if they have no symptoms or signs of infestation. (Pets do not require treatment.).

The mainstay of scabies treatment is the application of topical scabicidal agents, with repeat application in 7 days. The treatment of choice is permethrin lotion since clinical resistance has not been documented for isolated permethrin use.

I generally recommend my patients and their contacts with topical Permethrin 5% cream.The lotion should be applied over the entire body, including the face and scalp in infants. It should be left on for 8-12 hours and then rinsed. Reapplication 1 week later is advised.Please check with your physician if he shares my view and if can prescribe this drug to you.

If available, video dermatoscopy can be used to enhance the monitoring of clinical response to scabies treatment and allows for optimal timing of drug application.

This may minimize the risk of overtreatment, reduce the potential for side effects, and enhance patient compliance.

This is not a widespread technique. With video dermoscopy, a handheld device is used to illuminate, magnify, and record video of the skin.

Ty for info so I should reapply cream even though no other members have it? And about steriod can that work on scabies? I have a 17 month old who also showing no signs of it , I'm just lost and don't know much about this kind of stuff I tried looking it up for info but so many different things come up

Steroids are equally effective when used with Permethrin.

I understand your concern.

You need to be very patient to eradicate scabies completely from your house.

Yes, all other members have to apply this lotion even though they do not have symptoms.

Steroids are mainly used to control itching and inflammation in scabies.They are very effective.But they should be used alone.

Utilise the option of Video dermoscopy/VDS.This will produce encouraging results.Consult your dermatologist with regards to VDS.
