Suggest Treatment For Seborrheic Dermatitis

Question: Dear Doctor,
For at least one year I have suffered with what was diagnosed by my doctor to be seborrheic dermatitis. I have been using Sebiprox shampoo (15mg/g Ciclopirox-Olamine) as prescribed by my doctor. I shampoo about twice a week using this product and leave it on for 10 minutes before rinsing. Despite following this treatment for at least 6 months the symptoms have never gone away. They sometimes improve and other times worsen. At their worst my scalp is extremely irritated, red, sore and tender (of course accompanied by patches of scales and flakes). Important to note is that the symptoms seem to be very concentrated around the hairline on the forehead all the way to behind the ears. The pictures accompanying this consult where taken today and i feel they represent more or less the chronic state of my scalp (keeping in mind that it does get worse at times). Could you please provide your diagnosis? In the case that it is the same as my doctor's could you please provide possible alternative treatments and preventive measures? Could you please inform as to what factors may cause this condition (water quality, etc)? Many thanks in advance for your feedback.

Pityriasis amiantacea

Hi, Welcome to HealthcareMagic,

I have gone through your question and also the clinical image you uploaded.

At present your are suffering from Pityriasis amiantacea.It is an eczematous condition of the scalp resulting in adherent scale infiltrates and surrounds the base of a group of scalp hairs. Sometimes leads to hair fall also.
The conditions which leads to this condition is Seborrheic dermatitis of scalp, Scalp psoriasis, Atopic dermatitis and Lichen planus.

Confirm diagnosis can be made be dermatopathology.

All of these conditions can be effectively control but complete clearance takes long time.

Seborrheic dermatitis occurs in the area of heavy sebum production. There is excessive growth of a normal skin yeast (Pityrosporum or Malassezia). Cold and dry climate and stress also aggravates the condition. It has nothing to do with water.
The exact etiology of psoriasis is unknown, thought to be related with autoimmune process.

For control of disease continue washing your hair with Ciclopirox olamine.
I usually prescribe to my patients to apply Beclomethasone lotion or Fluocinolone acetonide lotion mixed with salicylic acid at day time.
Oxiconazole cream is very effective in controlling Seborrheic dermatitis. It has to be applied at night and washed in the morning.
