I have gone through your history in detail.
My XXXXXXX opinion is that you should go for genetic testing ,Most likely all these problems are interlinked to each other;Possibly it can be a genetic disorder which is causing autoimmune changes involving different organs(from bone to rheumatoid to dysautonomia).
A very detailed investigation would be required to rule out the cause of these changes,as unless we know the cause we can't treat it.
