Suggest Treatment For Severe Abdominal And Back Pain

Question: Hello Doctor, I am 32 years old male suffering from frequent fatigue, chills in the evenings along with severe lower back pain and lower abdominal pain for last 4 months. In the first month visited a orthopedic who suggested me for x-ray, ultrasound for abdomen and found it to be clean later assuming it to be muscle spasms and gave myoril tablets. The pain and suffering continued while havin certain spicy oily and caffeine foods and consulted a gastro, he ruled out appentics, hernia and put me on gastro medicines but the pain continued again. Last week Gastro doc suggested me for a CT scan, in the CT scan the radioligist found a swollen lymph node of 6cm x 3cm in the lower abdomen region, behind the navel and infront of spine, and mentioned it is likely tuberculosis. Could this swollen lymph node be the reson for abdominal and back pain ? I visited the general surgeon doctor who has put me on TB medications now. I have started taking my TB medications and it is my 7th day, the pain is still there, should I be taking pain killers to subside the lower back pain and abdominal pain ? Will these TB medications - 4antibiotics (isonaid, rifampcin, XXXXXXX inh) cure my swollen lymph node ?, how much time will it take to reduce the size of the swollen lymph node ?

I have gone through your query and understand your concern. I think that you need a second opinion. I would strongly suggest you to visit some chest clinic under the municipal corporation. They can provide quality treatment (after diagnosis) under RNTCP (Revised national tuberculosis control program). If it is due to tuberculosis, treatment with antitubercular medication should help in regularisation of the lymph nodes. But other possibilities need to be ruled out first.

How much time it would take can be commented only after proper diagnosis.

Sure Doc, will be going for follow-up to doctors and ask the same.

My question will there be pain when one suffers from TB and does TB medication cure it ? or does it require a pain killer once in a while ?And my TB is not in lungs, I did mention it is extra-pulmonary TB and a lymph node TB due to swollen lymph node in the lower abdomen behind the navel and in front of spine. I do not have cough, chest pain, my pain is mostly lower back and lower abdominal pain .... Please review my case again doc and provide your valuable thoughts

Thanks for asking back.

Pain is not necessary to be present for TB. The requirement of a pain killer entirely varies from case to case. I understand that your case is of suspected extrapulmonary TB but still you should visit the Chest Centres or Health Clinics under Municipal Corporation because that is the outlet through which all TB cases are supposed to be managed. I have already given this suggestion after careful consideration. They would be able to undertake the tests for confirmation.
