Question: Hello, my fiancé has been having abdominal pains for a week. He went to the ER twice this week and was sent home with some medication that has made it worse. This have been happening on and off since January. He has had CT scans, X-rays, blood work and a colonoscopy and all came back negative. His Dr says he is fine when he isn't. He can't keep food down when he tries to eat. Should he go back to the ER? His Dr is out of town for the next 2 weeks,

I have gone very carefully through your query and understand your concerns. I doubt if going to the ER once again would really help, since it has not helped earlier. Rather, let me myself try to help him out. Before going into anything else, I would need a description of the pain, its location, triggers and relieving factors. Tell me anything that would be relevant.

It can be that he is having gastritis or gastroesophageal reflux. If that is the case the pain is on the upper central abdomen or occasionally slightly to the left. Pain killers or NSAIDs can make the pain worse. Taking omeprazole once a day before breakfast would provide relief.

The description of the pain is a burning sensation, and the location of the pain is in the entire stomach area. The triggers are coffee and food period. Antacids and laying in a fetal position helps for awhile. He was given famotidine 20 mg and it makes him feel worse.

His problem seems to be due to gastritis or gastroesophageal reflux. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper.

In order to relieve himself, he need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fibre intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
