Suggest Treatment For Severe Abdominal Pain

Question : My wife has been suffering from severe pain in the stomach area . It started almost 11:00 hours ago.
She ate at noon , no supper.
She threw up 8 hours after she started feeling bad. The pain is slightly less when she stands up.
Can not lay down to rest. Took tums and one alkali selser .
She is Pre diabetic . I am not sure if it might be some sort of bacterial food problem . Not sure if it is nessary to go to ER. I am a microbiologist. Any thoughts ?

To recapitulate: Female/65 - severe pain in stomach ate at noon - no supper - threw 8 hours after feeling bad - pain is slightly less when she stand up - can not lay down to rest - tums and alkali selser - pre-diabetic - not sure whether bacterial infection not sure about going to ER.
My thoughts as per the history you have provided:
Since there is severe pain not allowing to lay down indicates the problem is more and needs clinical evaluation and examination by a Doctor, preferably a General Surgeon, followed by supportive investigations like X-ray of the abdomen in standing position, ultrasonography and tests of blood, urine as necessary to get a proper diagnosis.
This alone can give a proper plan for management.
It is always earlier - the better.
Please take her to the ER.
