Suggest Treatment For Severe Abdominal Pain
Pain other wise: Occasional sharp pains upper right abdomen. Occasional similar pain near below right shoulder blade. Occasional similar pain in upper left abdomen under the ribs. The pain just above the right hip is aggravated by movement.
Previous moderately heavy drinker of about 7-8 years. Average of a 6 pack a night often with more on the weekends. Recently cut back (1.5 weeks) drastically and only have a few drinks total Mon-Fri. I was also deployed (and dry) 2-3 months at a time over the last 7 years with about 4-5 months at home in between. Pain has been present for about 6 days. Had similar upper abdominal pains in the past the day after a night of heavier drinking..only would last about a day and it wasn't painful to the touch at the surface.
I had an abdominal xray taken ~5 days ago with no abnormalities. Nothing found during physical exam of the area. Nothing found in urine/blood tests except very slightly high kidney levels (described as likely just dehydration).
About 4 weeks ago I went to the ER with sharp pain (and pain to the touch) in my central lower abdomen and the same pain above my right hip up to the right flank. Subsided in ~4 days. Did not pass a stool for the last 1.5-2 days days which is very rare for me They took an abdominal CT scan and found nothing.
Stools and urine are normal and regular for me. Same with appetite, energy levels, etc.
Previous smokeless tobacco user which I also quit about 1 week ago.
Thanks!
FYI I am waiting on a referral for an abdominal ultrasound to further investigate.
Looks acid peptic disease.
Detailed Answer:
Welcome to Healthcaremagic.
Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
If i consider your CT Abdomen normal so, i virtually rule out gall bladder related issues, that fits in the description given by you.
Cholelithiasis associated with cholecystitis is the most common cause of the symptoms you have described, but that is picked up in CT scan.
So, my diagnosis would be an Acid peptic disease.
Since, your Ultrasound is pending, that would rule out ant remaining possibility of Cholcystitis.
My recommendations would be following for you:
1. To take a combination of Pantoprazole and Domperidone once a day.
2. To take Syrup Digene for symptomatic relief.
3. Avoid alcohol.
This is my personal opinion based on details available here. If you still have any other query, you may please ask me.
Stay Healthy.
