Brief Answer:
Are you an alcoholic?

Detailed Answer:
Thanks for asking on HealthcareMagic.

The level of liver enzymes (AST and ALT) are acceptable and I would not get quite concerned about them. Since you have fatty changes in your liver it would be a good idea to get your blood HbA1c level checked. It indicates the level of blood sugar of the last 3 months. Higher blood sugar levels are often related to fatty changes in liver. If you are a drinker, it might be a good idea to reduce or refrain from alcohol intake.

Brief Answer:
GERD

Detailed Answer:
Thanks for writing back. Your problem seems to be due to gastritis or gastroesophageal reflux. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fibre intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.

Hope that helps.

Let me know if I could help further.

Brief Answer:
More details required.

Detailed Answer:
I would still insist you to upload the report or to mention it in detail. I am looking to know the consistency, the pus cell count, the presence of undigested food particles and fat globules etc. Doing a culture of stool is useless. Stool of EVERYBODY contains bacteria. If it did not, you would not needed to bothered to wash your hands before touching food.

Talking about the anal itchiness, please mention have you ever detected worms around the anus or after being passed in stool?

I would await your response.

Brief Answer:
Try restricting wheat intake.

Detailed Answer:
Thanks for writing back. As expected, the pus cell count does not indicate presence of any infection. It is possible that you are intolerant to gluten and possibly lactose. You need to restrict intake of all wheat and wheat related products. You will find a list at: https://celiac.org/live-gluten-free/glutenfreediet/sources-of-gluten/
Try it for at least a fortnight and see if it makes a difference. Also restrict intake of daily products except for curd which can be taken.

Brief Answer:
Questions answered sequentially

Detailed Answer:
No, the time of the day would not affect the nature of stool. It will not alter the results. About rice, it is ok to be taken. However, as already suggested, you need to get the HbA1c level checked. Curd can be taken without worries. Fish can be taken. Take sea fish twice a week. You need not stop chicken but need to reduce the amount of oil used to cook it. However, you definitely need to restrict mutton intake.
