Suggest Treatment For Severe Acute Upper Right Quadrant Pain

Question : I spent 4 days in the hospital with acute upper right quadrant pain. I was told I possibly had kidney stones, and gallbladder sludge. I was given IV antibiotics and fluids for 3 days and sent home. I completed a round of oral antibiotics the following week. The surgeon decided not to take out the gallbladder. Week two has been the worst pain I have ever experienced. It wakes me out of my sleep when I can get to sleep. It's like having a XXXXXXX horse in my back that comes and goes. The pain has now radiated to the left flank area as well. It hurts to breath. I need to know what to do. My doctors offers no recourse except for pain meds, and I do not want to live my life on opiads. Ideas, suggestions?
I would also like to add that I cannot lye on my back or my right side without pain. Coughing or a hiccup causes a severe sharp pain in the same area(s). The pain is concentrated in my back ( both sides ) under the ribcage. I have not noticed a correlation between what I eat with the pain. The pain just NEVER goes away! The pain in worse in the early morning ( 2AM ) than during the day. Urination is not a problem. I stay hydrated and my urine appears normal. All my blood work came back with in normal limits. My potassium was low do to vomiting. I have had no vomiting for the past two weeks, but do run a low grade fever of 99.8. Help! I am ready for a bullet!

The pain you are referring to seems to be related to colitis attack or IBS. However, if I was your caring doctor would advise further tests to determine the right diagnosis:
- abdominal ultrasound
- urine analysis
- fecal examination
- stool culture
- colonoscopy/sigmoidoscopy

This is the first step toward finding the right diagnosis and decide for most appropriate treatment.

Opioids are good choice for pain relief, but not the final solution until tests will find it out.

I do not have any problem with diarrhea, bloating or any of the other symptoms related to IBS or colitis. Would either of these cause the severe pain under my right rib cage? My bowel movements are regular, there is no blood or mucus present. I have no cramping in my bowel region. All my pain is located on my right side and spreads to the left kidney area. I am more confused now than before.

I am sorry to get to know you got confused. Medicine has a lot of conditions and diseases which with an accurate medical/personal history and running some diagnostic tests will help find out your real problem.

If no problem with your bowels function and movements, then, I still insist to run further tests to rule out liver/kidney/pancreas/gallbladder problems.

My virtual judgment is based on what you are describing only. Based on your history, and with no lab test report, if I was your caring doctor would go for further examinations. However, If you have run any test till now, please send the reports in so I can analyze myself and help you specifically.
