Question : I have problems with severe anxiety at times, I blow everything out of proportion such as when I'm sick I always think the worse, my current psychologist has me on so many prescription pills and when I try to tell him how I feel he talks more about his problems, I do have depression, which is worse in the winter. I just don't think all these medicine help much. I had a severe bout with anxiety in August and September, I was having lots of tests and I was so scare I was crying all the time and thinking the worst, I called to talk to my pschcologist but you can only talk to the nurse and he told me there was nothing else they could give me. I need someone new.

I can understand your concern. In a lot of individuals with anxiety symptoms like over thinking and over analysing are seen. Some individuals develop hypochondriacal ideas like thinking worse on getting sick etc. You are currently on Quetiapine, Trazodone both of which are helping in sleep and also in reduction of symptoms. Duloxetine is a SNRI drug and helps in controlling anxiety and somatic symptoms. Bupropion is also an anti anxiety and anti depressant drug. Alprazolam helps to control the severe symptoms of anxiety.

Now coming to your current symptoms like crying spells on thinking worse, worsening depression etc, these are persisting even with medicines. In my opinion if these medicines are not working then other medicines like SSRIs especially Paroxetine or Fluoxetine can be used. These medicines will help to reduce the intrusive ideas like unwanted thoughts related to illness and thinking worse on getting sick. Repeat tests will reassure you but in the same time this results in compulsion to get the tests done again. High doses of these drugs will relieve anxiety and will also help in reducing the symptoms of depression.

In the events of severe anxiety try to take the doses of Alprazolam on SOS basis. This will help to beat symptoms and will reduce the attacks.

Apart from this try relaxation exercises like breathing exercise and progressive muscle relaxation exercises which will help to control the anxiety attacks. This will also build the confidence and the intrusive ideas will decrease.

So there is no need to remain distressed. Slight dose changes and changes in medicines will reduce the symptoms. Consult your doctor about the drugs I've mentioned and take new prescription of these drugs.

what does sos mean when taking the alprasolam?

You have been advised to take Alprazolam three times a day, but in events of severe anxiety you can use tablet (HALF TO ONE) as when needed. The drug is safe drug and in emergency situations more tablets can be used.
