Suggest Treatment For Severe Arthritis In The Knee

Question: My brother has been having trouble walking for several years and pain in the knees when he sat at a desk too long. starting 2 months ago, he went up a steep ramp and then the pain started being markedly worse. about a month ago, he could no longer get up and stand on the leg. at the hospital, they took xrays and he has been told basically he has no cartilage left and has severe arthritis in the knee. he is about 600 lbs and was told no orthopedic would even consider surgery. they gave him some shots and sent him to rehab. he is now able to stand but can only take a few steps and this causes agony in the shin. is there anything that can be done for him, we have already started a low carb diet to try to take some weight off.
