Question : This is for my brother who is 29 years old and shifted to Singapore 3 months back. I need your consultation as 3 weeks back he had severe pain in lower left back and not able to walk. Doctor was called at home. he have 4 injections and pain killers ( receipt attached) to relax the muscles. Its been more than 1 0 days now, he is still having difficulty in walking and pain. Needs to do exercise after every half an hour. currently he is taking lexotan 1.5 mg, arcoxia 120 mg, diclifenac and pengesic with Muscalax gel. Please suggest if any other medicine or course of treatment he needs to take ?

Brief Answer:
Non serious musclar low backache

Detailed Answer:
Hello xxxxxxx,

I went through the query and details attached.

It wrote that the backache was sudden after trying to lift a bucket of water. The medications were standard and the best treatment given anywhere for this kind of case and reason. Since your brother is very young his muscles will cope up sooner than a older person.

I think he has not taken necessary rest and the muscles are frequently going into spasm trying to protect his spine. He definitely needs physiotherapy and back building exercise.

If I were his treating doctor I would give the same medicine but definitely refer to physiotherapy for at least 6 weeks. This was lacking in his prescription.

I also think he should get an MRI of the spine to see if there is disc bulge. THe doctor has ruled out any weakness in the muscles which is a good sign for prognosis.

Let me know if there is anything that I can answer for you.

Regards

Thanks for answering the query doctor !!
One more thing, earlier he has pain in left and sometimes right lower back. Now days pain Is coming from particular area in legs which is causing difficulty in walking and moving. Is this all related to muscles only ? ? Also he is planning to visit phiseotherapist as well.

Brief Answer:
Possibility of sciatic nerve impingement

Detailed Answer:
Hello,

The symptoms described seems to be related to muscles of the back, back of thigh. They are run by sciatic nerve. Even if this nerve is impinged at some places the pain can occur. Hence I suggest an Spine surgeon evaluation plus MRI as well.

However physiotherapy is apt for both the situations mentioned. I still suggest to visit a physio. Without fail get a spine surgeon evaluation.
