Question: Hi Doctor,
My brother has got bipolar disorder .he talks a lot ,speaks very fast, spends a lot of money,smokes a lot,drinks, his eyes r droopy. does very heavy work , sleeps only for 2 to 3 hrs, avoids eating food, gets irritated on us. he goes out at 11 am n comes home at 6pm . n says he is charged n he likes it although it is dangerous. as he gets to do what ever he wants whn he is charged n can do 5 men work at the same time. his wife has left him n gone to her parents house .v r very much depressed abt him. yesterday v went to a G.P n he gave him n injection n gave him Trika sleping pill n some vitamin injection for 5 days. he does not want to go to a pschytrist. he only praises himself. kindly do guide. he was not like this before although he used to smoke n drink beer. but this has started since 10 days back.thank u.

olanzapine or resperidone

Detailed Answer:
Hello,

In such cases of mania we try to start atleast antipsychotics like olanzapine or resperidone. It can be given in food.

Resperidone is available in liquid formulations like risnia liquid, etc.
As they cause sedation ( sleep) they are preffrably given in evening time. One should not drive after taking them.

When I have such a patient, I start with 5-10mg olanzapine (mouth dissolving). And give a sedative like clonazepam 1-1.5mg at night.

There are injectables also available but they have to be administered forcefully...which might be difficult for you right now.
There are mood stabilizers like sodium valporate or lithium which are very effective but cant be mixed in food..

He needs either proper medicines at home or admission and managment at hospital. In no way he is going to improve without treatment and may become difficult with time.

You can ask other family members to admit him to psychiatric hospital forcefully.
