Question: For the past couple of days I have had severe bleeding when I poo. I have also been very tired the past week or so. Any ideas what may be wrong. 28yo female

If the bleeding is associated with pain while passing stools it's likely to be due anal fissure (small ulcer at anal opening caused due to tear secondary to hard stools ). It would be very helpful if you could provide me with the following additional information for me to be able to arrive at definite cause

1. Are you suffering from constipation /hhard stools ?

2. Is defecation associated with pain in anal region?

3. Is the blood mixed with stools or stools are coated with blood ?

4. Is there any swelling ot redness in anal region ?

5. Providing clinical photograph of anal region XXXXXXX be helpful

No constipation, stools are usually soft / sloppy, no pain, blood is both mixed in and coating the stool, no swelling. Bleeding is quite heavy and drips while sitting on the toilet. Sorry camera broken so can't take photo

Bleeding likely to be due to Piles

Detailed Answer:
Dear Ms

Going by description its likely to be due to Hemoorhoids(Piles). Exact diagnosis will depend on clinical examination which will include inspection of anal region and Proctoscopic examination.


I would suggest you the following

1. Drink plenty of water: 2-3 liter a day

2. High Fibre Diet: 2 servingfs of vegetable salad and 2 servings of fruits especially citrus fruit will ensure adequate intake of fibre in diet

3. Stool softner like Lactulose will be helpful


Further management will depend on clinical examination. I would suggest you to consult your primary care physician for further assessment.
