Question : Hi doctor my name is XXXX, my wife has chest pains and its interrupting her sleep we are lost and dont know what to do she has to work and has little rest...

Last night, we where up all night trying to find remedies etc. nothing worked, she was still in pain, she was taking medicine but the medicine kept making her feel like she couldnt breathe, so she stopped. Then she took another type of pill called tummyzen it helped for a bit but later last night she was in even worse pain, she was tosskng and turning, she said she felt like her heart was racing and it would get faster as she laid down, she was so desturbed she couldnt sleep we summed up the total amount to be about maybe a little over 30 minutes maybe less... she has to work today but she still cant sleep we are at a loss we've tried everything what can we do?

I understand your concern, and would explain that her symptoms could be related to gastro-esophageal reflux or gastritis. Hiatal hernia or a duodenal ulcer can not be excluded either.

The fact that tummyzen helps improve her situation, is another argument in favor of of the above causes.

Considering her age and her pain characteristics, I would exclude any cardiac issues, related to this situation.

Coming to this point, I would recommend consulting with her attending physician and performing some tests:

- a chest X ray study
- a resting ECG
- a fibrogastroscopy would be the test of choice to investigate for gastritis, gastro-esophageal reflux, a hiatal hernia or an ulcer.
- some blood lab tests (complete blood count, PCR and ESR) would be necessary to exclude inflammation as a possible cause of her complaints.

Meanwhile, I would recommend trying a PPI (omprazole, esomeprazole, etc.) instead of tummyzen.

What if we have already done that but they told us there was nothing wrong just that she has Acid reflux

Coming to this point, I would recommend taking a PPI before sleep and modify her diet.

A special bed against acid reflux with a higher position of the bed would help.

Or she can try sleeping with two pillows at night, in order to have a higher position of her head during sleep.
