Suggest Treatment For Severe Cough In A 2 Year Old

Question: My son is 2..5 year old, he had suffered with cought. During 3 month age he suffered with Broncolities. After maxmum time he suffered with cought. we go to the our Pedia. he suggest nebulization and medicine for 3 or 4 days. it helps but after 15-20 days again symtomes arise. During last year of august he had cought it was not stopeed by a soingle minute. we consult with pulmonologist, he suggest medicine and say he requires long time of nebulization then after that he is under nebulization. I we feel this is not 100% working, however he had cough.I consult two more doctor (regular visit in my office) they suggest long time nebulization has side effect .I am attaching medical prescription slip from first day to till date please review, and suggest me what is best for him. I am really very frustrated only to intake the medicine of my son.

Thanks for writing to us.
I can see you have been struggling to get relief for your son's cough.
I have seen the prescriptions and summaries you have attached.
The problem your child has is allergic bronchitis.
This needs treatment with medicines like budesonide as daily prevention.
You can use nebuliser or inhaler which is more effective for use.
Start with budesonide inhaler 100 mcg per puff.
For your child's condition I would advice to start with 2 puffs in the morning and 2 in the evening. Always use s spacer and mask ( available in pharmacy) with the inhaler device.
Remember to use it daily like brushing teeth without failing even for a single day.
Only with continuous use for atleast 6 months to one year the cough will settle. Then you can decrease the inhaler slowly over a period of 3 months or so.

There is no other medicine apart from inhaler or nebulisations.
If he develops cough inbetween you can nebulise with salbutamol three to six times a day for a few days till cough settles. Remember to continue the budesonide even during cough.
Never stop budesonide inhaler on your own if he feels better in a few days.
If budesonide alone doesn't work you can try combination like foracor t inhaler.

There is no side effects and THIS IS THE TREATMENT for his problem.

One more advice
Get allergy skin prick test from a allergy specialist in your area.
This will identify the triggers and you can avoid them, avoiding the triggers can help in faster control and cure.

Budesal 0.5mg TID (Nebulization) is quite enought for treating the issue.

If i nebulize when i face the issue is suggested or not Means when i feel his cough increased we will nebulize him by 4-5 days TID with budesal 0.5 mg. or without failing we should continue to nebulize.

How ever the cough is not well controlled we have continue nebulizing him by last 4 months and giving L-Montus kids at night.

Refer our Pulmonologist prescription is he treating well ? My office dr. told me he is giving too much medication so i got much confused and feel fear.

That's why i go to this portal for opinion.

Budesal during cough is good for a few days.
Once cough stops you have to stop it.
But budesonide or foracort ( as per your pulmonologist) inhaler or nebulisations should be given daily on all days with or without cough.

Your pulmonologist is treating well. Don't worry.
All you need is to stick to it and get repeated follow ups

Get skin testing done too.

How long this treatment is required for my son. ?
Only continuous nebulization is solution.?
during last three weeks i have countinue the Foracort but from last two days he has too much cough. Or i can say i m not getting good result......

please clear my doubt.

1. Continuous nebulisations is the only scientific option to control this issue. Other routes like syrups have tremendous side effects.
2. Treatment may be required for a year in some kids.
3. Identifying and avoiding the triggers by skin testing can help in better control which cannot be achieved by nebulisations alone.
