Suggest Treatment For Severe Dizziness

Question: Hi about three weeks ago I woke up not feeling well, I stayed in bed all day and night , woke up he next day and bent over to was my hair and got real dizzy, and the dizziness kept up most os the day I called my Doc and told them I thought I had Vertigo, they prescribed Ativert , 2 days later was worse and had an ear ache, woke up in the night soaking wet, called my Doc they put me on an antibiotic,it has been14 daays and I am still dizz and feel like a fever off and on, seen my Doc today and he does not see anything wrong

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. Ear ache and dizziness appear to be unrelated as dizziness started much before ear ache.
2. Dizziness appears to be much increased with change in posture, like when you bent over for the hair, or maybe during change in positions, like standing to sitting or sleeping to standing, etc. When done immediately, due to reduced effect of baroreceptors, which control the blood pressure, the blood pressure suddenly falls leading on to hypotension and dizziness. This is called as postural hypotension.
3. Increased fluid intake, Changing postures very slowly without any sudden change in positions should reduce dizziness. If on any antihypertensive medications, dosage reduction maybe necessary. Immediate blood pressure evaluation during postural changes, can confirm the diagnosis.
4. Vertigo usually can occur in any position and is not related to positional change. This here appears to be more towards postural hypotension.
5. Fever can lead to dehydration and hence more risk of dizziness due to hypotension, which is further increased by postural changes. Improve fluid intake along with nutrition. Protein rich diet will be helpful.
6. For ear ache secondary to infection, if fever is persisting, another antibiotic schedule may be necessary. If no fever, paracetamol or ibuprofen can be taken for pain.
