Suggest Treatment For Severe Fever In A Child

Question: My 10 month old kid, weight 9 kg, is running fever from 2 days. Fever increases everytime it comes back. Please suggest
Sequence of events.
1. Cough/cold for past 10-12 days
2. Till 10th we gave him Sinarest 2ml.
3. 12th aug, 2.30PM - fever 100.3F, gave Fevago -1ml
4. 12th Aug - 8PM, after consulting doc, fever-100.9F, gave fevago 1ml
5. 13th Aug - 12:05AM - fever - 101.3, gave fevago -1ml
6. 13 Aug - 5.30AM - fever -101.7 F, gave Meftal-p , 2ml
7. 13 Aug - 1PM - Fever shot up to 103.7F, gave Meftal-P 2ml.
8. 13 Aug - 7.40PM, Fever - could not check, gave meftal-p 2ml. after 25 mins of medicine it is still 102F

The worry is his fever is not coming down but increasing everytime. His doc told us that is viral. Also, he is congested due to cough and cold. Other medicine being given are Sinarest (2ml- TDS), Brozeet (2ml - TDS), Augmentin (3ml - BDS).

Please suggest what should I do? Is it ok to have such fever in viral. how long will it take to go away? Can I give steam?

I understand your anxiety and I must request you to bear with me while I give you my thoughts on what might be happening with your son.

First of all, I am inclined to disagree with the doctor's primary assessment that the fever might have been a viral one to start with. The reason is that your son had cough and cold 10 days ago, when it was probably a virus. However, the fever has come NOW ... after more than a week of the onset of cough symptoms. Not only that, it is serially increasing, and not coming down despite all the medicine you are giving him.

I am a little surprised that he has prescribed Augmentin, an antibiotic. This XXXXXXX is on account of the fact that antibiotics DO NOT WORK ON VIRAL ILLNESSES. If, as your doctor says, it is a virus, why use an antibiotic? Why is he using an un-indicated medicine? I would like you to pose this question to them.

Also, brozeet is used for a productive cough, and this is something you haven't told me anything about. Does your son have a fast breathing rate or a wheeze (the musical sound you can hear when you place your ear near his chest. The sound coincides with the breathing-OUT phase of the breathing cycle)? If not, brozeet may not work on him.

I am concerned about the fever rising gradually over the past 2 days. This is highly unusual, and warrants some blood tests, particularly a white cell count, a platelet count, and a check for malaria and dengue fever. No compromise should be made regarding this. Ask him to send you to the best place for testing, and not a place which is convenient for him or for you.

I was unable to find out where you are located. Please revert with more details such as your location, an idea of whether your locality has been seeing more and more cases of a particular illness (like flu, malaria or dengue, or even typhoid), about whether he has a wheeze or a faster than normal breathing, and about whether you will be prepared to get your son to a laboratory for testing.

I am more worried after your answer. We are located in XXXXXXX and in our society, we have had one case of Dengue but that was a couple of weeks back. Flu is persistent in our neighborhood. My son had a couple mosquito bites showing on his hand and foot day before yesterday and since yesterday he has fever. Can these be connected?

The reason doctor gave for antibiotics was that he has wet cough and which has continued for a while now. So she wanted to take care of that. Even today afternoon when I called her, she mentioned to wait for fever pattern till tomorrow and then go for a blood test. For brozeet, I am not so sure but in her examination report she mentioned "Chest B/L conducted sounds" , "throat congested". He still has cough which is slightly better as I feel. He even vomited once while coughing yesterday.

Also, I would like to mention that once we give him medicine (Meftal) he is fine in about an hour. To tell you frankly me an my wife are very confused.

Can he have cough in Dengue as well? and nasal congestion? Please help me in relieving my worry.

As your doctor is already in the loop for a blood test, please go ahead and get those tests done tomorrow morning. Ask specifically for a very good lab. Do the tests for typhoid, malaria, dengue and the whole blood count with platelets. This will be a bit expensive but worth it.

I can understand the prescriptions for the antibiotic and the Brozeet better after your clarifications. I would request you not to panic. Please trust your doctor. I am sure she is on top of things as of now. There is no harm in using Meftal, but I am personally a believer of only Brufen syrup and paracetamol ... you can use any one of these three, it does not matter. Avoid Meftal P as it is a combination of both mefenamic acid and paracetamol. Use only one ingredient medicines if possible.

Awaiting an update on his tests by tomorrow.

P.S. Give him just extra liquids and a light, easy to digest diet. Do not force him to eat or drink. Fever can reduce the appetite drastically.

We will see our doc tomorrow morning. In the meantime can you please help me with info on following too.

1. His fever drops to 99 or so after giving medicine. Is it ok?
2. The fever returns in about 6 hours. Can we give him meftal P after 6 hours instead of 8 as prescribed?
3. If fever increase starts decreasing by tomorrow,should we still go for tests?

Meftal P is only Mefenamic Acid.

The answer to all the above questions is yes. Yes, you may give Meftal after 6 hours, yes, it is okay for the fever to reduce to 99 or even below after medication and yes, you should still go for blood tests, because he is on round the clock fever medication.
