Question: I am a 45 year old man. Since 8years I have lost weight gradually, with people telling frequently that I have shrunk. I used to have fever earlier frequently but couple of years back I was diagnosed as having prostatitis and being taking flotral 10mg since then and I don't have fevers now. 4years back I had Mantoux test which turned positive, but the dr. Consulted told most of them turn positive, so no need to take drugs. I weigh 60 kgs and height is 5'10". What should I do ? Which is the test which confirms if at all I have tb correctly?

Brief Answer:
Mantoux test is not diagnostic of tuberculosis.

Detailed Answer:
Thanks for your question on Healthcare Magic.
I can understand your concern.
Mantoux test is old now. It has no diagnostic value these days. So if it comes positive, it doesn't mean that person is having tuberculosis.
So no need to worry for positive mantoux test.
To rule out tuberculosis, you should get done chest x ray, ultrasound examination of abdomen and neck.
If all these are normal then no need to worry for tuberculosis.
You are should also test for other causes of weight loss like diabetes, hyperthyroidism, dyslipidemia, malabsoption syndrome etc.
So get done HbA1C, TSH, free T3, free T4, lipid profile etc to rule out above mentioned diseases.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.

I have done chest x-ray, abdomen scanning, and other blood tests mentioned which have turned out to be normal except for lipid profile which is slightly on the higher side. Hb1ac was 6.

Yes dr. I think I do take things too seriously and that leads to stress. But I don't know what is the threshold. What constitutes stress. I am just worried about my weight and if at all anything is there to be diagnosed.

Stress is very complex subject.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
Honestly speaking, stress is very complex subject.
It is defined as ' A physical, chemical, or emotional factor that causes bodily or mental tension and may be a factor in disease causation'.
Now a days, almost all diseases have stress as causative factor.
So we should definitely rule out stress in your case.
So consult psychiatrist for the same.
Please let me know if you have already consulted psychiatrist.
I will be happy to help you further. Wish you good health. Thanks.

Brief Answer:
Another important cause is malignancy.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
Another important cause for unexplained weight loss is cancer (malignancy).
So you should get done routine tumor marker tests like serum PSA, CEA, AFP etc.
You also had prostatitis.
So we should definitely rule out premalignant prostate conditions.
So please let me know
1. Have you ever undergone serum PSA level?
2. Do you have any symptoms like burning micturation, increased frequency of urine, incomplete voiding sensation etc?
Please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further. Wish you good health. Thanks.
