Brief Answer:
Try Ibuprofen or Naproxen

Detailed Answer:
Hello ! Thank you for asking HealthcareMagic.
I have gone through your question and I can understand your concern.

In his case if the pain can not be managed with (NSAIDs like Tylenol) then treating doctor should prescribe an opioid drug (Tramadol) alone or combine with NSAIDs (Ibuprofen/Tylenol +opioid) [you may need prescription to obtain these medications].

Meanwhile, you can give him Ibuprofen or Naproxen these 2 are stronger than Tylenol and you can take them without prescription ( be sure he is not allergic first).
Ibuprofen 200 -400 mg /every 6-8hours after food.
Naproxen 250-275mg/once a day .

Wish your grandson good health.
Let me know if you have any further clarifications, I'll be happy to help.
Take Care!
