Suggest Treatment For Severe Head Pain

Posted on Fri, 13 Jan 2017
Question: Hello. My name is XXXX. I have this very weird and abnormal head pain that's been occurring for almost 2 years now. It's mostly on the right side of my head and it literally feels like a giant bubble or clot of some sort. I've gotten both a SPECT Scan and an MRI and although the SPECT scan showed some low chemical activity the MRI came back normal. Should I maybe get a CT scan instead? Because this is definitely not normal. About a week ago the sensations exacerbated by about 10x and it felt like my head was exploding and also was dealing with some confused thoughts at the time. These sensations are constant and never go away. Most of the doctors think its anxiety but I'm on anti anxiety meds so I know it's not that. Please help... :(

I hope those images were enough. Here's a description of the scan that I had done: The qEEG analyses were deviant from normal and showed dysregulation in bilateral frontal lobes especially in the left frontal lobe, bilateral temporal lobes especially in the left temporal lobe, bilateral parietal lobes especially in the left parietal lobe and bilateral occipital lobes especially in the left occipital lobe. LORETA showed dysregulation in the left parahippocampal gyrus and left fusiform gyrus. To the extent there is deviation from normla electrical patterns in these structures, then sub-optimal functioning is expected.

I'm not gonna lie to you I originally thought that the reason why I was having these issues was because I was still going through a prolonged withdrawal from Adderall. But it's been 2 years...I had gotten that SPECT scan done on 4/20/2016 so that would have been a little more than a year when I stopped Adderall. The Adderall is originally what I thought was giving me these sensations. Now I just don't know.

Brief Answer:
Medication details?

Detailed Answer:
Hi XXXXXXX
Thanks for being in follow-up and providing reports.

To be clear, suboptimal functioning in left frontal, parietal and occipital(all the lobes of brain) does not point towards a specific disease pattern. You might need a trial of medications. Can you please list the medications you have taken, and not proved to be helpful?

Right now I currently take Vyvanse 60mgs, Wellbutrin 450mgs, Seroquel 50mgs (just sometimes) and Ativan .5 mgs (just sometimes). Used to take Adderall 20mgs and Concerta 36mgs.

I switched to Vyvanse because it seemed more effective than Concerta. Stopped Adderall a couple of years ago since it was making me really depressed.

Like I said it basically feels like there's a knot or a bubble on the right side of my head. It's not rare that you'll find me moving my face in a twitch like motion because of this feeling, hence why I thought it was maybe still withdrawal. There will be some very rare occasions where the sensations spread throughout my entire head and will really intensify. It almost feels like a seizure is happening, only not quite. If I can find a way to describe it I guess the best way to put it is that it feels like a giant brain zap. It probably doesn't make a whole lot of sense but that's the best way I can put it....

Brief Answer:
Non-specific symptoms.

Detailed Answer:
Hi XXXXXXX
Thanks for being in follow-up.

I'm sure you'll find it hopeless again but I am sorry to say that the symptoms you are mentioning are non-specific for any disease especially when all the investigations have turned up negative.
These are likely to be stress/anxiety related.Stress may present with bizarre manifestations sometimes.
I'd suggest you to start doing mental relaxation/meditation on a regular basis and I am hopeful that the symptoms will improve soon.

Is there any chance that if I were to get a CT Scan that it may pick up something that the MRI didn't?

Brief Answer:
No.

Detailed Answer:
Hi XXXXXXX
Thanks for being in follow-up.

Not really XXXXXXX there is no such chance.MRI is far superior than CT.CT is used as a first line today only for the very few conditions like acute bleeding in the brain.

Hope that helps. If you have some further questions still,I shall be glad to have you in follow-up else please close the thread, rate it and write a review.
