Question: Hi,
I have underwent a lot of physical and mental suffering in the past. I took a toll on me. I am suffering now also. I feel emotionally stressed, severe head pressure, pain and i keep getting thoughts where people in the past pointed to me or ill treated me. i keep getting the same thoughts again and again. i am not able to sleep also. i am depressed and not able to focus on anything and feel stressed and exhausted all the time.

I can understand your concern. The symptoms you have mentioned suggest a depressive disorder but more details about the complaints are needed to confirm the diagnosis. You have mentioned that same thoughts repeat again and again.

I'd like to ask what thoughts you have in mind that repeat again and again?

If there is any try to suppress these thoughts?

Any other depressive symptoms use lack of energy, tiredness, fatigue, persistently low mood without any reason, any suicidal attempts, any history of substance abuse etc?

Since how much time you are having these symptoms?

Any other drug apart for Sertraline you have taken? Treatment response?

I had chronic pancreatitis in 2005 which caused a lot of pain and also a relationship breakup. I was depressed and physically very weak also. Many people gave me different responsibilities from work etc. which i couldnt complete because of the illness. I couldnt sleep also and was stressed badly as well. Since i couldnt complete them they treated me badly leaving me more depressed.
I keep getting the thoughts when they shouted at me. I used to feel that they are shouting at me all the time and such thoughts of attacking me came to me all the time. It caused me more panic.
I had chronic pancreatitis which caused lots of pain and physical weakness.
I tried to surpress these thoughts with meditation and relaxation. But it didnt work well.
I had all the symptoms like fatigue, low energy as you mentioned right from the age I entered into higher studies where lots of load of studying was there. I was stresed right from that age and tiredness. But these extreme symptoms were in the last few years(5 -6) and right now its really bad. No suicide attempts or substance abuse but have felt hopeless most of the time. Most of my thoughts are like people are abusing me.
Sertraline was the first one few years back. I was good for a while. I had positive thoughts after that but right now its not working and causing lots of headaches. Escalitaprom was the other one i ised recently but that didnt work as well. I am not able to concentrate on work so i was tried with Aderall for ADHD. It gave me extreme nervous symptoms. I use zolpidem tartrate for sleep now. 15 mg. It gives me sleep but once again day time is very tired and exhausted.

I am currently living in XXXXXXX . Winters here are very cold and no sunlight at all. I feel even more depressed during that time. Just for your information.

The symptoms suggest the depressive symptoms got precipitated after the episode of chronic pancreatitis and break up. Continuous work stress later aggravated the condition. The symptoms like fatigue, tiredness, low energy etc are classical symptoms of depression and should be treated accordingly. Escitalopram and Sertraline both are SSRI anti depressants and have almost same efficacy but are different in side effect profile.

As depressive symptoms are persistent even with medicines then SNRI drugs like Venlafaxine or Desvenlafaxine can help. Venlafaxine helps to improve serotonin and nor epinephrin neurotransmission so results in improvement of symptoms like lack of concentration, poor alertness, fatiguability etc. The drug takes time to show its effects and start showing effects in about 4 weeks. The drug is also a safe drug and is free of serious side effects.

For sleep problems if Zolpidem is not resulting in good sleep then in such case medicines like Trazodon, Mirtazapine or even benzodiazepines can help. Both drugs doesn't affect the sleep architecture so cause refreshing sleep. The former two drugs will help in depressive symptoms also. A good refreshing sleep will reduce the fatiguability and tiredness. You can discuss your doctor for prescription of these drugs.

Continue to do relaxation exercises and yoga. This will help to maintain confidence and will reduce the panic symptoms. For some time low dose benzodiazepines as Clonazepam or Lorazepam can be used to control the panic symptoms if used on SOS basis. Don't remain depressed, depression can be controlled with proper medicines and psychological methods. Remain calm and relaxed. Yes sun exposure is a treatment for depression. As there is poor sunlight so in such case light therapy can help. Keep the lights of your home and office switches on continuously and use bright light bulbs. This will improve the symptoms.

Hi Dr,
Thank You very much for the information. I have some more clarifications. I got chronic pancreatitis in year 2005 which was starting point of severe pain and depression as You said. I graduated from my 10th grade in 1999 and in my 11th and 12th grade a lot of study load was there like IIT and a heavy syllabus. I was 16 yrs at that time and my genetic changes in my body were starting. This period caused a lots of stress to me and I was exhausted quickly and became tired. I was able to study only for a small time and became fatigued soon. I had a lots of hair loss. From then on, stress always accompanied me. But I was not depressed at that time though only exhaustion and no pain in the body. Does this have any relevance? Or any ways to handle this? Severe headache in the head and neck always persists now. any specific techniques to relax at mental level?

The incidents and situation at the time of your studies doesn't have much relevance considering your situation but psychological factors and incidents happened even years ago might result in depression in future. The symptoms like tiredness, chronic fatigue etc are occurring due to the persistent depressive symptoms only. Treatment of depression will help to improve the symptoms and your will feel better. The headache, neck pain etc are due to somatic symptoms of depression most probably.

To handle this best thing that can be done is to take proper treatment of depression. Also start taking Vitamin B12, Omega 3 Fatty Acid supplements and this will help to improve your symptoms. Progressive muscle relaxation exercises, yoga will also help in relaxation and you will feel better in your symptoms.
