Brief Answer:
Not always

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

Heartburn is due to gastroesophageal reflux commonly called as acid reflux.
In a small percentage of individuals ( about 10 %), it leads to the development of a condition called Barrett's esophagus after a few decades of uncontrolled reflux.

This can in turn lead to cancer in a small percentage of patients ( the risk is 0.2% per year).

Regards,
Dr.R.K.
