sometimes the hardware plain breaks. this is generally as bad as it seems and in any case simple x rays will show if there is a problem.
Then, there can be scar. Physical therapy fixes it.
there can be an anatomical problem that the surgery just didn't fix. So, I've got someone who has a bone spur on one side of their knee and the surgery fixed the bone spurs that were (worse) on the OTHER side of the knee. He still has some problems.
Or, the muscles got smaller from lack of use around the time of the surgery. Pain also causes people to use them less. Then any use of the leg strains the poor muscles. Or the muscles can be less flexible, or scarred or less strong or their attachments can be strained.
Weight loss and flexibility and physical therapy would be the treatment BUT it hurts for months during the therapy.
