Question: My 86 year old father is suffering from a server nose bleed. He had been to the DR three times in three days and is bleeding again now. He needs an ENT.

1. This nasal bleeding is known as epistaxis.

2. Apply pressure (pinch the nostrils) for a few minutes to stop an anterior bleed. Apply an ice pack. Posterior bleeds require nasal packing, which can be done in an ER.

3. Check if there is any blood in the throat. His blood pressure should be recorded. If it is high, ask him to sit. Ask him not to swallow any blood as far as possible and collect it in a tray, vessel or towel.

4. An ENT Specialist can do a nasal endoscopy and cauterize any visible bleeding vessels. Medications such as ethamsylate and tranexamic acid help control the bleeds. A hemogram should be done to estimate the blood loss.

5. Check his current medication, if ant, for blood thinning medications and inform the doctor. If the bleeding is not controlled, get him to an ER.
