Question: XXXXXXX 1st I had a severe fall on hard marble surface at the entrance to a business mtg . someone had thrownup and I did not see it and slipped. Several immediate issues... sore neck... reaching out the right sholder , down right arm... last two fingers numb and sore wrist. ilanded on my right backside directly on the right buttuck dimple are adjacent to tail bone. Sore then and continues now. Numb fingers left hand from fall to now. Sore thumb too. I went to the emergency room in Vegas and they saw the list of symptoms and said" we will only treat 1 condition, the wrist shows a purple spot so we will do an exray.". they did and it was negative... they would not address it further thou I pleaded, and they would not look into other symptoms listed. They sent me back to the hotel and said to see my personal physician in pix when I returned from my business mtg. So, 1 week later I reached out to Mayo Internist and got in her. It has now been a week or so... she did an exam but, decided to watch and see if symptoms would go away. As weeks passed, and I complained I finally was seen by a hand specialist... it was discovered on my right side: partial tear right wrist tendon, partial tear in right rotor cuff. I had noted I was unable to type and get work done. There had developed highly sensitive pressure when typing on right forearm a..two inches below wrist. I broke out in shingles between examination and treatment decicided two weeks later. I was casted up to shoulder...1 week later..removed to check shingles site on right wrist..then recanted. 6 weeks later removed and splinter. And I have started PT for right wrist. In PT I complained that as we are moving forward with flexibility and strength exercises that when I was asked to begin a few minutes a day to type etc...the area in forearm became again very painful. Tuesday this week the PT(different) at Mayo addressed this discomfort and seemed to isolate it to the tendon adjoining the muscle ,under and between both tendons in forearm and felt "too" activity in therapy. There was no9thing they could do but rest further...a delay in recovery, Arg! Since the fall, the neck pain subsided and the shoulder area had shot of cortisone placed into the rotary cuff and has felt better . Left had.. I requested an exray on left thumb because it hurt still fromn the fall and no one had reviewed it or the tingly feeling in finger there. PA for orthopedic. Said the right thumb had a indication of "arthritis". (mind you it has NEVER hurt or been symptomatic) , so she drew the conclusion that the discomfort in my left thumb was from this! Agreed under pressure to exam it. Then gave mixed review... fracture in thumb(old) though I have never injured this before, and DJM. Then the radiologist has entered a record that the thumb had NO fracture. They as a result however then ordered a neurology EMG of left wrist to rule out carpal tunnel. Exam was not remarkable..conclusion everything is fine! But, tingly fingers are still symptomatic and are NOT fine. This occurred when I fell. 4yrs ago I was retreaded in a car accident and had 4567 cervical impingement. I had only had tingly fingers on RIGHT hand at that time. My hunch is that something is pinching in my neck but, they feel that because I am a diabetic type 2 that this odd feeling in hand is a neuroma unrelated to fall. Weird. Why would my left hand have this issue immediate onset a FALL. Neuroma develop over time. So they ran this Tuesday because I complained with my internist.... full blood workup to include A-C1...crp quant...ect. and I the doctor ordered a exray of both my fore/arm elbow ...amd right spine lower pelvic area to see if they missed something. Neither of these areas had been reviewed since the accident because all doctors felt the discomfort would pass over time. So... they were completed Tuesday. No feedback on exray yet, radiology is still surviving an option among them... and will chat with me tomorrow. But, the blood work is back... all really good EXCEPT: AC1 was 11.2. and the crp quant was 14. I have been on 800 mg of ibuprofen for 4 months every 6 hers until last week where I reduced it fearing trouble with my diabetes before Tuesdays exam. I looked up the other test not being familiar. Inflammation... no kidding....I have been saying I hurt all a long. No one has contacected me with the "next" step. I know stress can elevate my AC1 readings. I have been. Unable to type and or write with either hand for most of the time until I started kun therapy exercises. Writing is limited, but, I can now use my right middle finger to type for limited time. Then I give it a rest. Typing this note... looking down on tablet causes great muscle discomfort to my neck and it is throbbing. As the office I have purchased a VERDESK to lift my co9mputor UP to eye level and type looking directly across and this has helped. What do you think of my situation and continued discomfort?

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that your whole story has many loose ends and its very difficult to tie them without examining you but still I would try to help you out.

For the tingling and numbness in your hands both the factors are contributory . Since your diabetes is uncontrolled which results in neuropathy plus the injury you had led to tendon tear which would have increased the compartment pressure which could have resulted in the partial injury to the nerve .

For this taking Vitamin B12 supplement along with Pregabalin in combination would help out .

The arthritis of your thumb must be osteoarthritis of thumb as it is the most common ailment affecting the thumb at your age plus diabetics are more prone to have this.

Now coming to the shoulder thing since you had a tear it would have healed by now . Plus the cortisone injection would have accelerated the healing , its very effective in healing.

Other thing is you should stop taking ibuprofen and use it only when required . Let your test reports come and then I will guide about the further prospects.

Interesting...my internist. Just called. Nothing remarkable in xrays. However she is referring me to a physical MD @Mayo to review for soft tissue/ muscle injury not able to be seen on xrays. She is also referring to the Specialist for getting the diabetes back under control and perhaps the need of insulin to right it. This may be just a temporary change of additional meds than my current metphormin (2/5 00 me per day) and 1 my glimeride. Would you agree?

Yes i definitely agree with your treating doctor and strongly believe that sugar control is extremely essential to bring back quality of life. Diabetes is a systemic disease and uncontrolled one disturbs all the functions of the body and at this high level insulin happens to be the best choice.

Since your X rays are normal thats a good news , but you should take calcium and vitamin D supplements to make your bone stronger.

So I think you should go by what your doctor is saying.

when you suffer from other comorbidities specially like diabetes it complicates the healing process so thats the troubling issue . Please get your DEXA scan done to rule out osteoporosis as diabetic patients are more prone for it also and treatment needs to be started if you suffer from that.

Bone density is awesome...been an athlete for years. Goal...knock down Ac1. I am on the mend. Just a really bad fall and a more difficult recovery than first anticipated. Have a great weekend..ps: Thankyou for your insight. :)
