Question: I am having severe pain in my upper back near my right shoulder. The pain extends to my right arm. The pain increases when I lay down at night. I have tried heat which provide little relief.

The pain in the upper back extending to the arm can be related to a muscle spasm. Doing a sudden stretching and exercise can cause such a spasm to develop. You need to apply a local analgesic gel like Volini gel on the area after a hot compress. Do consult your doctor and get an oral muscle relaxant and a mild pain killer prescribed for relief. Avoid all painful activities till then.
