Suggest Treatment For Severe Pain In Right Shoulder

Question: Pain in the right shoulder, got worse within about a three hour window. Seems to be getting worse. Hard to lift arm without pain. Seems to be moving to upper right shoulder. No shortness of breath as of yet. No headach. Is this a possible stroke coming on.

I hope you could answer me a few questions about your condition for me to be in a better position to help you.

Do you have pins and needles sensation over your arm or hand ?

Any pain at the back of the neck that radiates to the shoulder ?

Are you able to lift your arm above your head ?

Do you have Diabetes ?

Do you have risk of stroke, chest pain or any heart disease ?

According to the history you have provided, you appear to be in severe right shoulder and upset back pain such comes on and off, possibly aggravated on shoulder movement.

There could be many causes for this condition like Muscle strain, Frozen shoulder which is common in Diabetics, Rotator cuff tear or arthrosis, Painful XXXXXXX syndrome, Sub Acromial Impingement,Cervical spondylosis, Slipped disc. Stroke hardly ever presents this way, unless you have developed weakness of the limb.

I would advise you to avoid lifting the arm above the head if it really hurts. Apply hot water bottle or warm soaks to the region of pain. Avoid bending the neck forward. Avoid caring any weight on the shoulders. Avoid working for long hours in front of the computer.

Adopt good posture while working and sleeping. Follow ergonomics at work. You cab take Tablet Diclofenac 75mg or 100mg + Thicolchicoside combination pain killer and muscle relaxant twice a day after meal. Alternatively you can apply pain killer gel of the same drug combination over the painful area. Avoid lying down or putting string over the right neck or shoulder.

Continue this regimen for 5 days and if you find no improvement you need to consult your Orthopaedic surgeon to arrive at the diagnosis after thoroughly examining you and performing necessary investigations like Shoulder or Cervical spine Xray and MRI Shoulder or Spins as necessary.

Not pins and needle pain, but intense constant pain. Like I said, this started about 3 hours ago and is getting worse. Cannot lift arm overhead without lots of pain. No diabetes. I do drink and smoke, over 35 years. Upper right shoulder and the muscel between shoulder and neck.

The pain you received could be due to a pinched nerve due to Cervical spondylosis or Slipped disc.

Kindly follow the precautions as mentioned earlier.

It would be wise to consult an Orthopaedician and get an MRI of the cervical spine as necessary to assess the degree of nerve pinching.

Do not move your arm about. You can use an arm pouch or shoulder immobiliser for now.

If you are having severe, unremitting pain, I advise you to consult your Orthopaedic surgeon as he would examine you and can possibly give you a steroid injection behind your shoulder in case it is a Frozen shoulder or arthritis.
