Question : I have severe pain in my shoulder joint. More uses more hurts. Seen doctors. Need physical therapy. Start next week. I need Otc med very strong

Since you have an acute pain in the shoulder you need to give your shoulder a rest. The best pain killer you can get OTC is Aleve or Motrin. It is best to take Aleve every 4 hours to have a persistent pain relief. Doing a hot compress on the shoulder will also help.
In case the pain is unbearable then do consult your orthopedician for a proper examination and stronger pain killers like Tramadol and muscle relaxant.
