                                            I need help my left shoulder really hurts if i move my arm or if i sleep on that side. If i take off my shirt it hurts
What could cause it?

It might be important to know if you had any trauma or not. With no trauma, doing an X ray might not be necessary.
Simply having a wrong or abnormal sleeping position, with tension or pressure on specific muscles can cause pain. I'd suggest you to start by applying a simple topical/local anti inflammatory drugs with a simple Analgesic (eg. Diclofenac). Starting with Tab Acetaminophen, generally sold over the counter (Tylenol) and topical Diclofenac could be good starting options.
In case this pain persists, you might need to see your doctor. Persistence might mean there might other chronic inflammatory disease, which might require specific tests. An erythrocyte sedimentation rate is a simple test, which if raised , measures the degree of inflammation.
For now, in case you have no recent history of trauma, you need not bother much. A plausible reason should be simple stress or tension on a muscle, and a simple analgesic and topical anti inflammatory drug should ok. In case this persists for over a week or so, you might see your doctor. For now, no worries.
