Suggest Treatment For Severe Pain In Wisdom Tooth

Question: Hello. My wife had been suffering from excruciating pain due to her wisdom tooth (lower right side). To make matters worse, no doctors were available since most of them are on religious holidays. We got hold of one dentist who was walking distance away with state of the art equipments at his chamber. He made an x-ray of the affected area (my wife's wisdom tooth) and said there was no need for a root canal but the wisdom tooth in question had been severely infected due to an improper previous filling.

Subsequently, on the same day, he performed a root canal on the wisdom tooth and also a temporary filling. This doctor sat down with my wife and explained the procedure he had done (he left a medicine in there to understand how far the infection had gone) and what to expect, but doubted the possibility of any further pain. He also said he would be off for a week due to the Eid-ul-Adha holidays.

Now, 24 hours after the root canal was performed, my wife has excruciating pain in the same wisdom tooth. I feel guilty because I was the one who took her to the dentist since I could not tolerate seeing her in pain.

She has been prescribed antibiotics and a powerful painkiller, but the painkiller has not helped a bit. She has to also nurse our six month old daughter regularly.

Now at this hour, Oct 6 2014, there are no dentists available due to Eid-ul-Adha and I don't know what to do for my wife. She has difficulty eating, and whenever her wisdom tooth comes in contact with another tooth, she says the pain is unbearable.

I have seen my wife tolerate her nose piercing without any tears. That's how strong she is. When I hear her mentioning that she's in severe (excruciating) pain, I have reasons for concern.

Would it be wise to extract the wisdom tooth at this stage? Also, what is happening to her wisdom tooth? Why is she in this much pain after one sitting of the root canal?

After carefully going through the history posted here,I can definitely advise you for removal of wisdom tooth.

Before I suggest further on this I would require few details.Please answer them for better suggestions on this.

1.Has she completed the course of antibiotics prescribed for her?
2.Does she has any missing teeth adjacent to wisdom teeth,in either upper or lower jaw?
3.Does she have any other symptoms other than pain,like swelling,numbness,pus in that area?
4.It would be nice if you can upload the radio graphic images here for view.

Thank you for your prompt reply.

Here are the answers:

1) She is on her third day of antibiotics, which was prescribed for seven (7) days (morning and night);

2) There are no missing teeth adjacent to the wisdom tooth, in either upper or lower jaw;

3) There is no apparent swelling at the moment, neither numbness - we are unable to ascertain whether there is any pus in the area since there are no doctors available to confirm this;

4) We are unable to upload the radio graphic images here as those are retained by the dentist at his office - we weren't provided the copy.

My wife tells me that the pain is so severe, she cannot touch her wisdom tooth even with her tongue. I do not know how else to explain. She is fearful of biting on food because of the pain!

It's Eid-ul-Adha here in XXXXXXX and all the doctors are on holiday for at least another 2-3 days.This was what her dentist had prescribed her before he left for holiday:

1) Furotil (Cefuroxime, 500 mg), for 7 days (day and night)

My wife is on her third day of this antibiotic.

2) XPA XR (665 mg) painkiller to be taken with Pantonix if she has any pain

Unfortunately, this has been of no help and she is still in severe pain.

My wife is also a nursing mother and has gastric.

Hello and thanks for reply.

your doctor's prescription is good and should be continued for the given period but there is a slight change required here.

Please ask your wife to take tablet Combiflam(400 mg brufen and 325mg paracetamol) for next three days,every six hourly.She should continue with her antibiotics for the given time period.Even a five day course is enough, but see that it should be taken regularly without fail.

The reason for having severe pain is that the tooth is acting like a closed chamber with the pus inside.This antibiotic will take care of the developed infection.

Any treatment can be done only when infection and pain are under control.Therefore please change the pain killer for now and see if it helps.

There is one more alternative to her pain,she can take local anesthetic injection at the site of pain.If you do not find any dentist,then you can also visit a physician for this.
Please let me know for further doubts.

Thank you for your prompt reply again.

Will the tablet Combiflam (400 mg brufen and 325mg paracetamol) have any effect on our six month old baby since my wife is nursing her and our baby is totally dependent upon the breastmilk? Regards.Also the fact my wife has gastritis. Will Combiflam be any problem taking into account of her gastritis and the fact she is nursing the baby? Will she need to take pantonix before taking the Combiflam? When should it be taken? After meal or before? Thanks.

Hello and thanks for reply again.

Please do not worry about combiflam,I had taken due consideration before advising the drug.It is totally safe to be used for three days even in lactating mother.She can breast feed the child and then take her food.After that she can take her medication.

Now regarding her gastritis problem,yes Combiflam has a brufen component in it, which is not advised in case of severe gastritis.But even in this case she can take after food and yes she can definitely take this medication along with pantonix, if required.

Keeping the severity of pain in due consideration and for a short duration of three days, Combiflam would give better results.

Once the infection and pain is under control,please get the tooth extracted.This would be my advise,as retention of third molars is not at all necessary.It is considered as vestigeal organ.

It becomes important to save third molars only when adjacent teeth is missing in the arch, for the purpose of bridge.
