1. Is the pain present in the same tooth which is root canal treated or is the pain present in any other tooth?
2. If the pain is in tooth other than root canal treated tooth, then is the tooth decayed or is it loose?
3. Please specify your age, sex, and do you have any medical problems or are you on any medications?
4. Do you have any known allergies?
