Suggest Treatment For Severe Vertigo While Having Hyperthyroidism

default
Posted on Mon, 17 Apr 2017
Question: I am 57 years female suffering from hyperthyroidism since 2011. I have been monitoring thyroid levels by way of regular tests and medication. The relevant reports have been uploaded for your ready reference. I am very regular in taking medication.
There have been extraordinary elevation of TSH levels 76.53 (19/04/2013), 14.51 (03/06/2016) & elevation upto normal range 2.11 (04/05/2015) at times during which I was prescribed thyronorm alongwith thyrocarb.
I also had this episode of Positional Vertigo (BPPV) in XXXXXXX 2016.
I want to specifically state that
1.     Whenever I take thyronorm, I feel uneasy which is unexplainable.
2.     Post BPPV, there is a continuous but mild clinking ( XXXX ) in my head when I go to bed though it subsides after a while after I ignore it.
3.     I am confused regarding the use of thyronorm.
4.     Should I take iodized salt or plain salt.
You are requested to please give advice so that I may regulate my thyroid levels and lead a normal life. I want to humbly add that i have taken voluntary retirement in july 2013 because i was not able to cope with my job with my hyperthyroidism. Presently i am a pensioner and therefore i have to limit my expenditure of treatment.

with regards

Brief Answer:
You are on both Thyrocab and Thyronorm?---I am also a little surprised

Detailed Answer:
Hi XXXXXXX welcome to Health care magic,
I have seen the reports. Please clarify----
You were diagnosed with Hyperthyroidism about 8 months back?
You are sufferring from BPPV since?
You are on both Thyrocab and Thyronorm?---I am also a little surprised at this--one reduces high thyroid levels (Carbimazole) and the other raises it (Thyroxin).
Why are both being given? This has led to Hypothyroid levels.
If you have Hypo (less) thyroid levels--you should take iodised salt.
I would treat you as a Hypothyroid now and stop Thyrocab, frequently monitor TSH levels and adjust dose of Thyronorm.
Regarding BPPV, can you tell me what they are giving you for it at present.
Please revert back so that I can help you--which I am sure I will be able to.
Regards

I am extremely grateful for your kind response Mam.
I would like to submit the following:
I was diagnosed with hyperthyroid way back in 2001. I have also undergone radio active Iodine tests if you refer my older reports (19/09/2012) which has also suggested hyperthyroidism.
I have uploaded 4 reports (starting from 2013 till date in 4 different files).
Recently, I was prescribed tab carbimazole 5mg for 7 d along with 25 microgram thyronorm for 7d by the family clinic on 7/02/17 as against the test report of t3-244.87, t4-10.9 and TSH-0.06 done in DiagnoLabs on 3/02/17. But I want to confess that I DID NOT TAKE THRONORM as is seemed illogical and I continued with 7.5 mg carbimazole daily on my own. I again got tested on 10/03/2017 and the levels reported are as t3-149.03, t4-9.08 and TSH-0.008 (Kotgirwar path lab). After these reports i have increased the dose to 12.5 mg carbimazole daily on my own. So right now i am not on both Thyrocab and Thyronorm.

These are the latest levels which suggests hyperthyroidism (Please correct me if I am wrong). And if I am correct should I take iodized salt or non-iodized salt. I need your help to interpret these results. Please suggest the medication with correct dose.
I also fail to understand why both Thyrocab and Thyronorm are being prescribed and as a matter of fact I have developed a phobia against Thyronorm because it makes me really uncomfortable.
This BPPV episode occurred only during XXXXXXX 2016 and now I am fine. I am not taking anything although I have been advised to take stugil tab if vertigo does recur.
I submit the above facts with all humility and without any hard feelings against anybody and request you to please help me. I just want to lead a healthy life and get the value for my money being a pensioner.

Brief Answer:
ok you have hyperthyroidism and need only Carbimazole.

Detailed Answer:
Hi, ok you have hyperthyroidism and need only Carbimazole. I think you are on right track. Only test your thyroid levels every 15 days, and maintain in normal range. Increase/decrease by 2.5 mg if needed.
No iodised salt.
You can revert back.

Thank you Mam.
There are some more queries:
1.     I was advised to take calcium earlier. Should I take some vitamin/calcium/iron supplement?
2.     My daughter is 19 years old and she is staying with us (Me and my husband) and studying. I want to know that if we stop using iodized salt, will there be some adverse health concerns for my daughter and husband? I want to add that we are non-vegetarian and take well-balanced nutritious diet.
3.     Is there any side effect of continuous use of carbimazole? If yes, how it can be mitigated?
4.     Are there any restrictions for particular food/vegetable/fruits in my case?
Thanking you again,
With regards,

Brief Answer:
take calcium only if levels are low.

Detailed Answer:
Hi, take calcium only if levels are low.
Your husband and daughter can continue with iodised salt.
For hyperthyroidism--no restriction on any fruits and vegetables.
Carbimazole--it can lead to mild side effects like nausea, vomiting, joint pains to serious ones like Bone marrow suppression (rare).
You should monitor blood counts regularly when taking it.
