Brief Answer:
Slough needs to be debrided; antibiotics essential

Detailed Answer:
Hi there.
I have seen the photograph provided & Yes there is slough & debris in the area & it is secondarily infected.
Please go across to an ER/Surgeon/GP anyone easily accessible & get the slough removed or 'debrided' as the medical term implies.
You will also need a course of antibiotics both oral & topical & this too will require a prescription hence my suggestion.
Alternatively, you may try cleaning the area yourself using a dilute 1:10 hydrogen peroxide lotion & /or a Sitz bath but given the location of the ulceration, it will be a difficult task. I suggest one visit to the doctor & then follow up at home.
Sorry I am the bearer of bad tidings but this is the best honest opinion I could come up with & do hope it points you in the right direction.
Good Luck & Godspeed.

Dr. Rodrigues,

I was afraid of the answer I would receive. I am seeing a dermatologist in the morning and thought knowing might ease my anxiety. I so appreciate your expertise and hope you are not traumatized by my ugly behind! I am wondering now about the pain involved in this procedure, as it is as painful as it looks. The shingles showed up the week after my mother passed away. Many, many thanks for the great things I know that you do for people. May God richly bless you and your family.

Brief Answer:
keep your chin & bottoms up..will be fine soon.

Detailed Answer:
Hello again Ma'am.
I apologise for having played the devil's advocate but felt it was important I am honest with you at this point to stress that this needs intervention.
It need not be painful in the right hands as a local EMLA (anaesthetic cream) can be applied to the region 1 hour before start of procedure & /or an injectable local anaesthetic to dull the pain & then debrided.
I am sorry for the loss of your mother & the stress et al may have precipitated the eruption of the herpes zoster.
I do hope this clear picture is positive & has eased your anxiety.
As for the trauma of your ugly behind, don't worry...if I may say so, in my line of work I have seen much worse & this is easily sorted in a week or so tops!
I am sure you will get back soon to your good looks et al..do stay in touch & thanks for the good wishes too. Cheers!
