I would explain as follows:

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

I carefully passed through your question and would explain that his symptoms seem to be compatible with platypnea.

This is a rare condition, when shortness of breath is relieved by lying down position and worsens during sitting or standing up. It is the opposite of orthopnea which is the most common condition in cardiac patients.

You should know that the main causes underlying this disorder are hepatopulmonary syndrome or an anatomical cardiovascular defect increasing positional right-to-left shunting, called Abernethy malformation.

It is not related to any cardiac disorders.

So, I would recommend performing some further tests:

- a chest X ray study and respiratory function tests to exclude possible lung disorder
- liver function tests and an abdominal ultrasound to examine the blood flow in the liver vessels
- measure blood oxygen saturation in standing up and lying down position.

You should discuss with his doctor on the above issues.

I would also like to review his cardiac ultrasound report for another professional opinion.

If you have any other questions, please do not hesitate to ask me again!

Kind regards,

Dr. Iliri
