Question: I am 60 +and I am taking Veltam Plus for last 22 days. recently i am facing dry orgasms. even if i masturbate, I get erection but I have no orgasm - nothing comes out. I get the feeling of getting orgasm, but no liquid comes out

Thanks for your query,based on the facts that you have posted it appears that you have been diagnosed to have enlarged prostate and taking combination of Tomsulosin and Dutasteride (Veltam Plus) to improve your urine flow and facing problem of dry orgasm .This is termed as retrograde ejaculation..

During normal process of ejaculation bladder neck gets closed where as sphincter urethra located at apex of prostatic urethra relaxes This leads to entry of semen in forward direction in to urethra which is then pumped out through urethral opening .

However this mechanism is deranged due to Tomsulosin .I would explain in detail how it happens.

Tomsulosin is selective alpha adrenergic blocker that relaxes smooth muscles located in bladder neck and prostate .This results into bladder neck to remain wide open hence entire semen enters back into bladder but there is no forward entry of semen into urethra ,resulting into dry ejaculation.Hence one gets sensation of sexual climax (Orgasm ) without normal ejaculation.This is called as Retrograde Ejaculation.

This ca be confirmed by microscopic examination of urine passed immediately after intercourse which will reveal presence of sperms in urine.

Do not worry .Your ejaculation will resume to normal once you stop taking Tomsulosin.
