Question: What are the possible concerns when someone develops an bad headache in the sinuses, congestion, for over a week...and now has one eye that doesn't open as wide as the other. The eye that doesn't open is the same side as the sinus headache?

Could be because of frontal sinusttis.

Hi,
Welcome to health care magic,

If some one has a bad sinus headache because of acute sinusitis then it could be possible it has spread as a result of infection of frontal sinus (the sinus which lies above the eye).

There are several factors which contribute to sinusitis includes,

Bacterial or viral infection, especially after you have a cold
fungus,allergy,changes in temperature,chemical fumes etc.

He might not be able to open the eye because of conjunctivitis which has spread due to that frontal sinusitis.

It can also spread to your teeth, upper jaw and other parts of your head.
