Suggest Treatment For Skipped Heart Beats And Arrhythmia

Question: Hi Doctor:

I have cardiac pvcs. Have been to cardiologists and have had cardiac workup which they told me I have pvcs or ectopic beats. Had stress test earlier this year. Lately I have been under extreme stress with my mom being sick, plus working many hours. I may be sitting down then all of a sudden my heart flip flops flutter and skips every beat for such a long time. It is so scary and annoying. It feels like its jumping out of my neck and throat. This has been happening mostly at night when I am in bed making it hard to fall asleep. Called the cardio and he told me to take the atenolol only as needed and not every day (1/2 tab at 12.5mg). I also take Xanax 025mg for the pvcs cause the Atenolol tends to lower my bp. The last few nights they have been so bad I took the Xanax with the Atenolol and finally got a regular heart beat otherwise it was skipping every beat. During the day I may get flutters. When I am busy or walking I dont feel anything mostly when I am relaxed. Could you please explain this whole senario. Should I be worried? How can I help myself to deal with this? Thank you for your help.

Dear Mrs,

From your description it seems you have premature beats (extrasystolies) caused by stress, which are usually benign. Both Xanax and Atenolol are good for controlling such arrhythmias. If Xanax alone can't control arrhythmias adequately, and you can't tolerate 12.5mh Atenolol because of your low BP, I would advise you to take Atenolol 6.25mg together with Xanax.
Also remember that your arrhythmia is benign and the only harm is inconvenience and don't worry much. Try to be much in the fresh air, eat food reach in Mg. Also you can do Yoga or meditation, to relax your nervous system.

Thank you for your quick response. I need to convince myself that these are benign they are just scarry. Do you think taking Atenolol as needed would suffice as opposed to daily and if so when should I take it on the sudden surge of a pvc?

The PVCs are benign in this case. You can take Atenolol, when you feel your arrhythmias become more frequent. But if you feel inconvenience with every other PVC, you can take low dose Atenolol with Xanax every day.
