Suggest Treatment For Solitary Mediastinal Lymph Node

Question: I'm currently in the USA visiting my girlfriend for a month (I live in England). The day before I left to fly out (18th August) I was told that during a routine spinal MRI I had for some back pain and stiffness they found an enlarged mediastinal lymph node (2cm) around the T1 region. I was recommended to follow this up with my GP. I originally planned to do this when I came back in a month but then decided to get it out the way before leaving so I made a same day appointment on that day and saw my GP. He sent me for a bunch of blood tests (FBC, Liver, Kidneys, Inflammation and some cancer-specific ones like LDH and ESR). These have all since come back completely normal.

I kind of forgot about the problem until this morning (26th August) when I got a phone call from my dad saying that I have had an appointment with a Haematologist come through for this Friday (28th August). I can't get home in time for this so my dad had the appointment moved back to the day I arrive back home - which is the 16th September in 21 days time.

The reason I am posting today is to ask if I should consider this situation serious and should I be cancelling my trip to come home sooner as a matter of urgency? My blood tests were all fine and I don't believe I have any other symptoms. I don't have night sweats, fever, rashes or itchy skin. The Dr also said that there were no other palpable enlarged lymph nodes in my jaw, neck, groin or armpits when I saw him. I don't feel any more fatigued than usual and my appetite is normal. I have lost a stone in weight over the last few months but I have been actively trying very hard to lose weight recently as I'm overweight. A private abdominal ultrasound I had last month said that my spleen and liver were of a normal size. I have no problems breathing and have no particular pain anywhere.

I was diagnosed with ME/CFS at 22 years old and this is a long-term illness which does affect my day to day living quite a lot. I mention this as it is not unusual for me to suffer with enlarged lymph nodes relatively frequently due to this (it is one of the main diagnostic criteria and most frequent symptoms). I also take Low Dose Naltrexone for the ME/CFS which I believe can modulate the immune system and could possibly cause lymph node enlargement.

There is no history of any sort of blood cancer in my family either.The only other symptoms I have are daily bloating and water retention in my chest/abdomen (had this for almost 4 years and been told it is nothing serious) and fatty liver with mildly elevated triglycerides and cholesterol. There are no recent changes in my long-term ME/CFS symptoms either.

Obviously, I do not want to put my health at unnecessary risk however I do not currently have the money to change my flight at the moment (it would cost me £200 to change it) so I am quite worried as to what to do and I'm not sure how serious this situation is. The haemotologist appointment made for me seems to have been a 2-week fast-track one, so this is what has spooked me. I will be back in England in 21 days. I would be very greatful if anyone could offer any advice. As of this very moment there doesnt seem to be any noticeable symptoms at all so the only current issue which has my Dr wanting to rule out Lymphoma is a single enlarged (2cm) lymph node found on a spinal MRI scan.

What do you think? Is an extra 19 day wait to see the Haemotologist possibly putting my health at a huge risk for
a single enlarged lymph node? The accompanying appointment for a CT scan to look at all my other nodes in my chest/abdomen
and pelvis will only be put back by 4 days due to my holiday as that was a 3-4 week wait so not as urgent I believe.

I would second with this decision as abscence of all the symptoms and normal baseline tests rule out any need of urgency in the whole mater, though I would still suggest you to arrange the appointment for CT or MRI of chest, abdomen and pelvis for other lymph nodes assessment, sooner after your visit in September.

At this moment, on the basis of one solitary lymph node, there is not enough reasons to suggest that you shorten your tour.

The theory relating to immuno-modulatory medicines you mentioned, being responsible for increased lymph node, is partially true until all other causes are ruled out.
