1. That would be a tongue bite.
2. The delicate lining tends to break down causing a ulcer. the action of saliva cause the white ring like appearance. This is normal.
3. Treat it like a common apthous ulcer and it should heal on its own soon. Continue the oral rinse. Use medical grade glycerin or amlexanox gel locally as a barrier to speed up healing and reduce pain and discomfort. Diet should be bland.
4. You may also get it examined if it persists for another couple of weeks as direct observation is the best way to judge if any intervention is require. If you upload a close-up image, it will help.
